Welcome and thank you for your interest in this study! This survey is part of a research study intended to examine the relationship between leisure physical activities and fatigue in multiple sclerosis (MS). It is being conducted at Mount St. Mary’s University in Emmitsburg, Maryland. Your participation is much appreciated and will hopefully help patients, family members, and medical professionals better understand the symptoms and experiences of persons with MS. Your participation is voluntary and you may withdraw your participation at any time. We believe that participation should be anonymous, so we will not ask you to provide your name, address, or any other identifying information. The information we collect from your participation will be kept confidential.
